What to expect
On your first visit to a Great Lakes clinic you will be evaluated the therapist with whom you made the appointment. The purpose of this assessment is to fully and clearly understand the patient’s problems and the physical basis for the symptoms that cause the patient to complain.
The therapist will then prescribe an individualized plan of treatment that will include home exercises, education, manual therapy, modalities, and gym work. Treatment will begin the first day.

When to arrive
For you first appointment we like you to come 15 minutes prior to your scheduled time to fill out paperwork. If you have a physician’s referral or results of diagnostic imaging, we ask that you bring those as well. To facilitate promptness we can email you your intake forms in advance.
What to wear
Bring some loose comfortable clothing. For example, if we are examining a lower extremity injury (hip, knee) bring a pair of shorts. A sleeveless top is suitable if you are attending for a shoulder injury. Bring any braces or orthotics that have been prescribed.
What to bring
Bring with you any referrals, xray or ultrasound results, list of current medications for your current condition as well as any other conditions, health insurance card. Bring the name of your family physician as we will communicate with them.
If you require reading glasses, bring them, as there will be intake forms, and outcome measures that will need to be completed as part of your assessment.
